Draft Order - Appointment of Gordoncare
GordonCare Services For Separated Families:
Incorporating: The Parent Order Program and Children's Contact Services

Generic Paragraph

1. For the purposes of Supervised/Facilitated Contact; Transition for Supervised/Facilitated Changeover; Supervised/Facilitated Change Over  and Parent Orders Program (delete where applicable)  the husband/wife (mother/father/carer/ALL RELEVANT FAMILY MEMBERS):
 
1.1 as soon as practicable do all such things necessary, including signing of documents, to complete the application process for GordonCare Services for Separated Families which may also incorporate the use of the Children’s Contact Service, where deemed appropriate for the children.
1.2
 as and when required satisfactorily complete all recommended parts of the GordonCare for Children family specific service design of the Services for Separated Families Program.  This may include some or all of the following: participation in the parent group, children’s group, individual sessions for parents and children, child focused mediation (these program components are covered under court privilege and/or Children’s Contact Service (this program component not covered under court privilege).
1.3 be responsible for all fees (if any) associated with the involvement of GordonCare for Children.
1.4 abide by all reasonable requests of any person employed by GordonCare for Children.
1.5 successfully complete the “Contact Transition Program” (if required).
 
Supervised (Facilitated) Contact
2. The (“Parent the Child Spends time with” ) have supervised contact with the child(ren) (insert name and date of birth- not necessary if details already in order) for two hours each (alternative) weekend on such day and at such time as nominated by GordonCare Children’s Contact Services ("GordonCare").

Transition from Supervised (Facilitated) Contact to Supervised (Facilitated) Changeover
3. The Parent/Carers’ participate in a graduated plan which is child focused, assisting the child(ren’)s progress from supervised/facilitated contact to supervised/facilitated  changeover at the  Children’s Contact Service.

4. For the purposes of contact pursuant to paragraph (refer to paragraph detailing contact) contact changeover take place at such time and on such day as nominated by GordonCare Children’s Contact Services.

Parent Orders Program
5. The (details of Parents and or Carers and Children ) participate in all recommended      components of the Specific family Service Design of the GordonCare for Children – Services for Separated Families – Stand By Me - Parent Orders Program ("GordonCare").
